Know your Patient’s Rights!
Under the new proposed rule from the Centers for Medicare & Medicaid
Services (CMS), called IMPACT ACT, healthcare providers that include hospitals,
rehabilitation and long - term care facilities, and home health agencies
must provide you (patient or a designated caregiver) with a completed
discharge plan before you leave. The plan must include detailed information
with discharge instructions, completed medication reconciliation updated
for your current needs. Discharge Planners must not only provide you with
such updated information but also provide this information to your provider
of choice. THis can include your primary care physician, your home health
/ home care agency, other community based services.
The important change is that now as a patient (or caregiver) you are given
more rights to participate in the discharge planning process. You have
the right to select a provider of choice based on the performance and
quality outcomes. You can easily find Performance Results data on the
